The incident occurred at Primary Children's Hospital, where the child had been admitted for seizure monitoring due to autism. Medical personnel became concerned and alerted the authorities after observing distressing behavior from the father. On Sunday, officers from the Lehi Police Department were called to the hospital by a nurse who had previously reported a case of child abuse. The nurse informed the police that there were concerns raised the night before, but they had not received updates from the Division of Child and Family Services regarding the ongoing case. Due to the lack of information linking to a prior investigation, the police initiated their own inquiry into the situation. Witnesses indicated that the father had been seen on video appearing to strangle his son.
